Medical Report Information

When creating a medical report the Subject of the report should always be Name | Type for example John Doe | RTC. Please Use the report template below for all medical reports or on the Reports page of the MDT search for "Report Template 2025". It is suggested that you take a screenshot of the Medical System so you can remember what injuries the person had.
